Patient's Responsibility:
Giving Dr. Walter all the information necessary to assess your needs is our first goal. You will fill out the information sheets you downloaded from the first page of our website. If you have trouble printing these pages, you can come in 15 minutes before your scheduled appointment time and fill them out in the comfort of our reception room. Bring a picture ID (a drivers license or a work ID) because your records are kept electronically and your insurance ID if you wish us to file a claim. You don't need to have insurance to be a patient, but if you want to use that benefit we need the card.
What the Patient Will Receive:
- The full attention of Dr. Walter while he reviews your information in your presence, asking you about your health
history and the history of your condition.
- A full examination including vital signs, neurological testing, functional testing, and chiropractic testing.
- X-rays, if indicated, of the problem area will be exposed, developed and interpreted.
- Treatment may be offered that is safe and appropriate for your complaint at this early stage
- A determination of the next step in your care will be given by the doctor.
- Friendly, courteous, timely service by a well-qualified staff
- A follow-up appointment time that meets Dr. Walter's recommendations and your schedule.
After you leave:
- Dr. Walter will prepare a full report of findings to present to you on your next visit.
- Dr. Walter will prepare a treatment plan and explain that to you on that visit.
- The staff will prepare an explanation of our policies and your financial obligation for the treatment plan.
